One Matters Awards: Call for Applications

To Apply for the Award, the Annual Conference Will:

• Nominate a congregation.
• Collect information and documentation (listed below).
• Complete the online application.
• Receive the plaque and check.
• Acknowledge and celebrate the congregation during annual conference, through an article in the AC newsletter, or any other method chosen by the AC.

Information Needed for Application

• OMA delivery date (if different form AC date)
• Pastor’s full name, address, and full name of awarded congregation
• Reason why this congregation is celebrated
• How the AC is planning to deliver the award, acknowledge, and celebrate the congregation (i.e., AC session or other method.)
• Annual conference GCNO (GCFA number).

Annual Conference Documents Required for Upload

• Direct deposit authorization form, completed, dated, and signed. (You will be able to download the ACH during the application process.)
• Copy of a voided check
• Completed W-9

Note: Annual conferences that applied for the OMA in 2022 and uploaded current documents, do not need to re-upload them. All other annual conferences need to upload all documents listed above.
